Use this form to apply to have your physician education activity certified for Maintenance of Certification (MOC) Lifelong Learning & Self-Assessment (Part 2) Points (continuing education credits) with the American medical boards listed at the bottom of this page. This form ensures your activity meets the requirements for each specialty board.
The American medical specialty boards listed at the bottom of this page have developed MOC programs to help their diplomates enhance & assess their clinical knowledge and practice skills, and these specialty boards have begun collaborating with the Accreditation Council for Continuing Medical Education (ACCME) to automate the process for the boards confirming their diplomates’ participation in physician education activities that meet their MOC requirements. HCA Healthcare Continental Division can provide MOC Part 2 Points for CME activities by registering them and their participants with the ACCME, where the medical specialty boards can confirm participation by its diplomates. HealthONE is accredited by the ACCME to provide continuing medical education (CME) for physicians.
Your activity must also be approved for AMA PRA Category 1 CreditsTM by HCA Healthcare Continental Division via our CME Credit Application in order to receive MOC Part 2 Points through the Office of Continuing Medical.
HCA Healthcare Continental Division charges a Course Certification fee per year for the administration & documentation required for us to provide MOC Part 2 Points for an activity.
Submissions to Specialty Boards
Participation info for this activity will be submitted to the ACCME, and ultimately the specialty board, for MOC Part 2 Points. HCA Healthcare Continental Division will not submit participation info to the ACCME until all documentation is complete for this activity including this MOC Points Application, the CME Credit Application, and the CME Closeout form.
